The memorial can be found in the parish churchyard, Gwytherin and takes the form of a plain obelisk on a square plinth, with two steps. surrounded by railings. The inscription and names are on the front of the plinth. There are seven names listed for World War 1.

Private 53005 [SDGW] or 53605 [CWGC], 1st Battalion, Royal Welsh Fusiliers. Killed in action 30th March 1917. Aged 19. Born Doifrwynog, enlisted Llanrwst, resident Gytherin. Son of Thomas and Winnifred P. Davies, of Ty Bont, Rhyd Lanfair, Bettws-Y-Coed, Carnarvonshire. No known grave. Commemorated on ARRAS MEMORIAL, Pas de Calais, France. Bay 6.

Private 35811, 14th Battalion, South Wales Borderers. Died in United Kingdom 5th April 1916. Born Llanwrst, enlisted Wrexham. Buried between the Church and Mortuary in SS. ELERI AND JAMES CHURCHYARD, GWYTHERIN, Denbighshire.
