The memorial is situated in the grounds of St John the Baptist, Axbridge. Itn takes the form of a wheel cross, of rough hewn granite, surmounting three-stepped stone base, each step of the base bears the names and inscription.

DURBIN	Arthur	Private 4402, 1/4th (T.F.) Battalion, Hampshire Regiment. Died of wounds in Mesopotamia 19 January 1917. Aged 19. Born Wells, Somerset, enlisted Axbridge. Son of Henry George and Mary Durbin, of West St., Axbridge, Somerset. Formerly 1818, Somersetshire Light Infantry. No known grave. Commemorated on BASRA MEMORIAL, Iraq. Panel 21 and 63.
FRANCIS	Harry	Private 20296, "A" Company, 6th Battalion, Dorsetshire Regiment. Died of wounds 31 March 1918. Aged 19. Born and resident Axbridge, enlisted Weston-Super-Mare, Somerset. Son of Mr. and Mrs. Robert Francis, of Axbridge, Somerset. Buried in DOULLENS COMMUNAL CEMETERY EXTENSION NO.1, Somme, France. Plot VI. Row G. Grave 37.
HANCOCK	Percy	Private 5168, 1/4th (City of Bristol) Battalion (T.F.), Gloucestershire Regiment. Died of wounds 26 July 1916. Born Axbridge, enlisted Bristol. Husband of A. Hancock, of West St., Axbridge, Somerset. Buried in ST. PIERRE CEMETERY, AMIENS, Somme, France. Plot II. Row E. Grave 1.
JAMES	Archibald Arthur	Private 10223, 6th Battalion, Prince Albert's (Somerset Light Infantry). Killed in action 26 August 1917. Aged 22. Born and resident Chaple Allerton, Somerset, enlisted Bristol. Son of John James, of Moor Lane, Axbridge, Somerset. No known grave. Commemorated on TYNE COT MEMORIAL, West-Vlaanderen, Belgium. Panel 41 to 42 and 163A.

LINTERN	Charles Edward aka Charley	Private 47620, 17th Battalion, Manchester Regiment. Killed in action 2 July 1917. Aged 38. Born Blagdon, Somerset, enlisted Axbridge. Son of William and Thirza Lintern, of Rickford, Burrington, Bristol. Formerly R/4/140556, Royal Army Service Corps. Buried in RENINGHELST NEW MILITARY CEMETERY, West-Vlaanderen, Belgium. Plot III. Row F. Grave 1.
OFFER	Richard Ambrose	Able Seaman Bristol Z/31, Howe Battalion, Royal Naval Division, Royal Naval Volunteer Reserve. Killed in action between 4 and 6 June 1915. Aged 20. Born 24 June 1894. Son of Mrs. Ann Jear, of High St., Banwell, Weston-super-Mare. He was resident at Blanche Cottage, Silver Street, Wrington, Somerset. He was Church of England. before enlisting he was a farm labourer, unable to swim, heioght 5 feet 5 inches, chest 32½ inches. complexion fair, dark brown hair, dark grey eyes. Enlisted 2 November 1914. He had a fiancee, Miss Ann May, of 166 Noish Alliayton, Bayford, Dorset. Buried in SKEW BRIDGE CEMETERY, Turkey (including Gallipoli). Plot II. Row C. Grave 1.
ROE	Percy [Victor]	Private 72069, 11th Battalion, Cheshire Regiment. Killed in action 1 June 1918. Aged 18. Born Axbridge, enlisted Tredegar, Monmouthshire. Son of Alfred and Beatrice Roe, of 2, The Myrtles, Axbridge, Somerset. No known grave. Commemorated on SOISSONS MEMORIAL, Aisne, France.
WEARE	George Coomer	Private A/587, 1st Battalion, Candian Infantry. Died 25 July 1916. Aged 39. Born 15 October 1875 in Axbridge, Somerset. Son of the late Charles and Charlotte Weare, of Station Street, Axbridge, Somerset. Farmer by trade. Unmarried. Previously served 9 years in the Royal Navy. Attested nad passed fit 16 January 1915 at London, Ontario, Canada. Enlisted aged 34 years 3 months. height 5 feet 5 inches, girth 36 inches. complexion fair, eyes blue, brown hair, religious denomination C hurch of England. No known grave. Son of John James Carp and Eliza Lewis Carp, of Bristol; husband of Ruth Carp, of Clifton, Bristol. Master Mariner. Awarded the Distinguished Service Cross (D.S.C.). Buried in BRISTOL (CANFORD) CEMETERY, Gloucestershire. Section OO. Grave 103.
FISH	Edward Roy	Stoker 1st Class D/KX 111786, H.M.S. Gloxinia, Royal Navy. Died 12 March 1943. Aged 22. Son of Charles Edward and Elsie Fish, of Axbridge, Somerset. Buried in SIMON'S TOWN (DIDO VALLEY) CEMETERY, Western Cape, South Africa. Naval Allotment Row D. Grave 28.
GAGE	Frederick James	Lance Corporal 5670726, 5th Battalion, Dorsetshire Regiment. Died 27 June 1943. Aged 26. Son of Frederick Robert and Rosina Gage; husband of Sophy Irene Gage, of Axbridge. Buried in AXBRIDGE (ST. JOHN THE BAPTIST) CHURCHYARD, Axbridge, Somerset.
GRIMSTEAD	Percival George	Private 5680956, 2nd Battalion, Wiltshire Regiment. Died 10 August 1943. Aged 33. Son of Mr. and Mrs. E. Grimstead; husband of D. I. Grimstead, of Axbridge, Somerset. Buried in CATANIA WAR CEMETERY, SICILY, Italy. Plot II. Row B. Grave 50.
HUCKER, MM	John Kingsman	Serjeant 1071689, 19 Field Regiment, Royal Artillery. Died 4 February 1944. Son of Henry and Laura Hucker; husband of Frances May Hucker. Awarded the Military Medal (M.M.). Buried in ANZIO WAR CEMETERY, Italy. Plot IV. Row E. Grave 8.
